Voluntary Inquiry to the Office of Student Disability and Accessibility Services

Qualified individuals are entitled to reasonable accommodations under the Americans with Disabilities Act (ADA), as amended, and Section 504 of the 1973 Rehabilitation Act. Accommodations are determined on a case-by-case basis. All information submitted to the Office of Student Disability and Accessibility Services (OSDAS) will be considered confidential. If you would like to start the registration process, or have a general inquiry, please complete this inquiry form and forward it to OSDAS. Returning this form does not constitute a request for accommodation or guarantee an accommodation. A follow-up meeting will be scheduled to complete the registration packet. In order to receive services, documentation verifying a disability is required as well as the completion of a self report.
